There are two ways an order form can be submitted:

   First, you can request that an order form be mailed to you by clicking on the button below. An order form and catalog will be mailed to you. You fill out the order form and mail it, with your payment, to the address on the form.    Second, you can fill out an order form online by clicking on the button below. An order form will appear which you can fill out. The payment will be paid by cedit card, which is at the bottom of the form.



Instructions for filling out Order Form

   Complete the sold to: and ship to: sections completely. We must have a name, address (with street address, state, and zip code). Include a telephone number or e-mail so we can contact you.

   Complete the body of the order form giving the correct stock number, quantity desired, description of item, and price.

   Texas residents must include an 8.25 percent sales tax. All shipments outside of Texas do not have sales tax.

  After you have listed all the items you require, calculate the sales tax (if any) and shipping charges. Then total your order. We have the right to adjust your total for errors on prices, sales tax, and shipping cost.

   Please give any shipping instructions you want that are different from our policy. 

   Last, complete the credit card section. We will use this infomation to pay for your order. No order will be shipped unless we have the credit card information.
